Items to Provide to EA Prior to Opening

EA requires the following to be submitted prior to approval for store opening: 

• Completed Franchise Fact Sheet 
• Pre-Opening Pictures that show the completion of construction of the store (10 days prior to scheduled opening week) 
• Certificate of Occupancy / health department certification, whichever is needed locally for opening (10 days prior to scheduled opening week) 
• Signed Lease 
• Certificate of Insurance 
• Completed Weekly Debit Authorization form for the electronic funds transfer (EFT) of weekly royalties and fees
